The Network developed a platform for designated hospitals to share learning on the Monday.com. This space has been created to enable shared learning across the TYA Principal Treatment Centres and Designated Hospitals. Its purpose is to strengthen clinical governance by facilitating the sharing of themes, quality improvements, and learning from incidents across the Network.
Examples include:
- Involves shared care
- Has learning value
- Caused harm or serious risk
- Highlights pathway or system improvements
- Related to communication between DH and PTC
- Good governance and learning outcomes to share
- Other (to be specified)
Please note:
Must have gone through local incident review process and relevant to TYA specific systems / processes
This is not a first‑point escalation route
No patient‑identifiable information should ever be submitted
